NOTE:

THREE DOWNLOAD CHOICES

The first is a .pdf file which must be completed by hand after downloading.

The second, also a .pdf file, can be opened after downloading and completed by computer. Users of Macs should open with Preview to be able to save; otherwise, users of Acrobat, PC or Mac,must complete the form at one sitting.

The third is a Microsoft WORD file which, after downloading, can be opened and completed on your computer (assuming you have MS Word on it).

In all cases, a signature is needed before submitting the application to the Guidance Office.
